Target group or special population served: Homeless and runaway youth ages 14 to 21

Number and size of teams: One team - two outreach workers

Items provided at contact: Food, personal hygiene supplies, transportation assistance, water

Drop-in center: 1244 E. McDowell Road

Other services available through the agency: Transitional shelter, case management, counseling, employment referral.

Other important information: Youth contacted through outreach will be assessed for Home Base Transitional Living program, if interested.

Items provided at contact: Water, food, hygiene items, socks (when available), transportation to drop-in center.

Drop-in center: 420 E. Roosevelt St., Monday through Thursday from 8:15 a.m. to 7 p.m.; Friday from 8 a.m. to 6 p.m.; Saturday and Sunday from 9 a.m. to 3 p.m.

Other services available through the agency: Shelter and showers, case management, counseling, food, clothing, referral for education, employment, assistance in locating permanent housing, medical care.

Other important information: Youth ages 10 to 17 may be eligible for Tumbleweed Group Home.

Seriously Mentally Ill

Drop-in center: Will transport to Safe Haven

Other services available through the agency: Case management, psychiatric services, assistance with shelter, referral for medical and other benefit services, transportation services

Other important information: Path Outreach operates Monday through Friday, 8 a.m. to 5 p.m. After hours and weekends contact the Crisis Network.
