Fire Prevention General Inspection Permits & Fees


Permits -- The following are the permit applications currently available on-line.

This list is not all inclusive

Commercial Day Care Application
An operational permit shall be required to operate a Commercial Day Care Facility. Prior to the issuance of the operational permit a thorough fire inspection must be completed on an annual basis and upon change of ownership. The operational permit fee for a licensed facility for less than 50 persons shall be $135 and $270 for more than 50.

Behavioral, Developmentally Disabled or Correctional Facility Application
An operational permit shall be required to operate a Behavioral, Developmentally Disabled or Correctional Facility. Prior to the issuance of the operational permit a thorough fire inspection must be completed on an annual basis and upon change of ownership. The operational permit fee for a licensed facility is $135 plus $10 per bed.

Assisted Living Facility Application
An operational permit shall be required to operate an Assisted Living Facility. Prior to the issuance of the operational permit a thorough fire inspection must be completed) when initially opened, when there is a change of owner and every three (3) years thereafter. The operational permit fee for a licensed facility is $135 plus $10 per bed.

Assisted Living Facilities are classified as R-3, 1-10 beds and R-4, 11 or more beds.
No inspections shall be scheduled or conducted until all related fees are paid in advance.

Assembly Permit Application
Outdoor Assembly (An event with an anticipated occupant load of 500 or more persons or confine 50 or more attendees by the permanent or temporary installation of barricades or fencing)
Examples:
Haunted House
Vehicle Display (To display, compete, or demonstrate a liquid – or gas – fueled vehicles or equipment in buildings)
Public Assembly (To temporarily change an occupancy into a Public Assembly)
Christmas Trees in Public Buildings
Trade Shows / Exhibits

Burn Permit Application
Bonfire / Control Burns
Hot Work (Welding, torching, and open flame cutting)
Propane Burn Off / Flare-up
Sweat Lodge

Tent Canopy Permit Packet
Tents / Canopies

Note: Tent permit submittals shall be 10 business days in advance of the event. A fee of $25 per day progressive penalty shall be assessed for permit applications not submitted 10 business days prior to the event.

Zoning approval for location of tent/canopy is required. Tent/canopy plans will be reviewed by the Fire Department at a later time, but must be included with the packet.

Operating permits required for businesses

Your business may require permits from the Phoenix Fire Department if you have or perform any of the following:
  • Operate a place of assembly. A public assembly is a facility with an occupant load of fifty or more persons. (i.e. restaurants, bars, night clubs, meeting halls, or churches)
  • Conducting spraying or dipping operations utilizing flammable or combustible liquids.
  • Conducting cutting, welding, use of open torch, brazing, glass blowing, or similar operations.
  • Manufacture more than one gallon a day of organic coatings.
  • Operate a motor vehicle fuel-dispensing station.
  • Operate a stationary lead-acid battery system.
  • Store, use, and handle hazardous materials (i.e. compressed gasses, flammable and combustible liquids, corrosives, oxidizers, water reactive, and cryogenics). Please click here for the Hazardous Materials Permit Amounts and Fees.

Please make all checks payable to the “City of Phoenix”.  Credit cards may be used at the Fire Prevention counter located at the above address or called in.  Please note outdoor assemblies, trade shows, bonfires, control burns, tents, and canopies require a site map and plans with the application.   Most permits require that the application be submitted ten (10) business days prior to the event date.
