Early ballot requests can be made with the City of Phoenix by calling 602-261-VOTE (8683) or obtain the request form online at <a href="http://lists.phoenix.gov/t/1407646/4119508/9432/3/">Phoenix.gov/Elections</a>.</p></td></tr></tbody></table><p>Voters who wish to vote early in person rather than by mail, may cast an early ballot during business hours at the City Clerk Office on the 15<sup>th </sup>floor of Phoenix City Hall, 200 W. Washington St., from 8 a.m. to 5 p.m. any weekday through Friday, May 17. Accessible voting devices that permit voters with disabilities to vote independently are available at these locations.
