Public Records Request Form

Arizona Public Records Law (A.R.S. 39-121) requires a description and statement of purpose be provided to request public records. Although requests are not required to be in writing, the City of Phoenix provides this form to ensure timely and accurate fulfillment by aiding in clarification, workflow and reporting. If you need assistance filling out this form, please call (602) 262-7177 or visit the City Clerk Department at City Hall, 200 W. Washington, 15th Floor, Phoenix, AZ 85003. Submission of this form or verbal confirmation of its contents will be required to process requests.

Each request is processed in the order in which it is received. Various factors affect processing time, including the request’s size and complexity, and nature of the records sought.

Purpose

Seeking public records for a "commercial purpose" means the requestor's purpose is to use the records, directly or indirectly, to receive monetary gain, such as by selling or reselling all or parts of the records, or by using the names, addresses, and phone numbers in the records for solicitation. Media requests are not commercial purpose requests.

A person who obtains a public record for a commercial purpose without indicating the commercial purpose or who obtains a public record for a noncommercial purpose and uses or knowingly allows the use of such public record for a commercial purpose or who obtains a public record for a commercial purpose and uses or knowingly allows the use of such public record for a different commercial purpose or who obtains a public record from anyone other than the custodian of such records and uses it for a commercial purpose shall in addition to other penalties be liable to the state or the political subdivision from which the public record was obtained for damages in the amount of three times the amount which would have been charged for the public record had the commercial purpose been stated plus costs and reasonable attorney fees or shall be liable to the state or the political subdivision for the amount of three times the actual damages if it can be shown that the public record would not have been provided had the commercial purpose of actual use been stated at the time of obtaining the records. ARS § 39-121.03(C).

View the Public Records Fee Schedule. A deposit may be required for sizable requests.

You will be notified when the requested item(s) is ready for pick up or review. Once notified, the item(s) will be available for 30 days. If the item(s) is not picked up or reviewed after 30 days, it will be returned to its normal retention schedule and purged pursuant to City’s retention policy.

The information submitted in this contact form may constitute a public record as defined in A.R.S. § 41-151.18, and may be subject to inspection by or disclosure to third parties upon request for commercial or non-commercial use in accordance with A.R.S. § 39-121 et al.

Any documents produced pursuant to public records requests made under A.R.S. §§ 39-121 and 39-121.01 may be protected by the Federal Copyright regulation. The use of such documents without the copyright holder’s consent may be a copyright infringement subject to civil sanctions and criminal penalties.”
