Fill out the application and submit with an electronic plan for review. Use Administrative Procedure (AP)5.1 Plan Checklist for your plan submittal to eliminate or minimize corrections required. If proposed work is in an archealogical area per UPIMS, applicant must complete and submit an archealogical information sheet with the permit application.
