Cultural Facilities Work Order
Cultural Facilities Work Orders are handled by the Facilities and Operations Division, where all requests for maintenance of City of Phoenix cultural facilities are created. Once created, the Work Order will be assigned to the appropriate person.
Arts and Culture hours of operation are 8:30 a.m. - 5:00 p.m.
After-hours maintenance is available by calling (602) 262-6732.
Work Order Categories
Priority 1 (P1): Emergency - Immediate Response
Priority 2 (P2): Within 24 Hours for Response
Priority 3 (P3): Scheduled Work Orders/Within 72 Hours for Response
P1 Emergencies
If this is an emergency or an after-hours request, please don't submit a work order. Please contact the Public Works Work Control at 602-262-6732 for an immediate response, and please also text Romeo Rabusa at 623-262-7681 with your emergency or after-hours request.

Requests submitted here are processed throughout the day in the order they are received. Your request will be routed to Arts and Culture's Facilities and Operations team when you click the Submit button below.
