Homeless ID Project

Visit Acacia Library every 2nd & 4th Thursday of the month. First come, first serve. Get help with AZ ID Replacement and Birth Certificates.

To replace a lost or stolen AZ ID or AZ Driver's License. For a 1st time AZ ID or Driver's License, please bring what you can: birth certificate, Social Security card, AHCCCS insurance card, and ID cards from any other state. A valid out of state DL is required to switch to an AZ DL.

Birth certificates will only be issued if required for obtaining an Arizona State ID or for housing purposes. If a birth certificate is needed for housing, a supporting document from a case manager or housing facility will be required at the time of application.
