The Phoenix Police Department has released a Critical Incident Briefing (CIB) video that includes audio, visuals and information related to an officer-involved shooting (OIS) which occurred on August 19, 2025.
The incident began near 20th Avenue and Northern Avenue when Phoenix police officers responded to a trespassing call at a local restaurant. The caller told the 911 operator that a man had entered the restaurant after being told to leave and had threatened a customer. The caller described the man as a white male in his 50s, wearing a white shirt and blue jeans.
Multiple 911 calls were received regarding the incident, including one from a restaurant employee and another from a customer inside the establishment.
When officers arrived on the scene, they found the man standing outside the restaurant, armed with a hatchet and a chisel. Officers issued multiple commands, but the man did not comply and continued walking toward them.
One officer deployed a 40mm less-lethal launcher, striking the man with limited effect. As the man continued walking towards the hospital, the officer-involved shooting occurred.
Officers rendered aid until the Phoenix Fire Department arrived and transported the man to a nearby hospital, where he later died.
No other individuals were injured during the incident.
The officer involved is assigned to patrol in the Desert Horizon Precinct and has been with the department for seven years.
This incident is the subject of a criminal investigation to be reviewed by the Maricopa County Attorney’s Office.
This incident is the subject of a criminal investigation being conducted by the Major Incident Division of the Department of Public Safety. It will be reviewed by the Maricopa County Attorney’s Office. It is also the subject of an administrative investigation being conducted by the Phoenix Police Department.
Conclusions about whether the actions of the officers are consistent with department policy and the law will not be made until all facts are known and the investigation is complete.
