- Eye exam every plan year. Copay may apply.
- Standard Contact Lens Fit and Follow Up. Credit allowance may apply.

- Single Vision Lenses. Credit allowance may apply.
- Progressive or Bifocal Lenses. Credit allowance may apply.
- Trifocal, Lenticular Lenses. Credit allowance may apply.

Did you Know...
The Buy-Up Vision plan includes a Lasik Reimbursement of $200, which is available for any provider, in-network or out-of-network, payable once per lifetime. All members also have access to discounts for Lasik-related services of up to 50% when they use a provider under the QualSight program. Contact QualSight for assistance in locating a provider and scheduling their service. Members are eligible for the discounts in addition to the $200 benefit.
