Nursing, daycare, assisted living

​Arizona Department of Health Services Licensed Facilities


All State Licensed Facilities including Nursing Homes, Adult and Child Care Facilities, Behavioral Health, Medical Facilities, Hospitals and Assisted Living Homes are required to have a Phoenix Fire Department operating permit. Inspection paperwork will be provided at the end of the inspection process for submittal to the State for license renewal.  

Fire Permits are not transferrable and must be applied for by new owners or if the business name changes.

Assisted Living Facilities

- Inspection required every 3 years

Behavioral Health, Correctional or Developmental Facility

- Inspection required every year

Day Care Facility for Children or Adults

- Inspection required every year

Medical Facilities 

- Inspection required every year
