Cable TV Comment Form

The City of Phoenix licenses companies to provide cable television services in Phoenix. Currently, there are two such services. They are: Qwest Broadband Services, Inc. (CenturyLink) and Cox Communications Arizona, LLC. (Cox).

The city's Information Technology Services department accepts complaints from Phoenix residents regarding the two licensed cable television providers.

The city acts as a liaison between these cable TV providers and its residents to resolve complaints. The city encourages residents to attempt to resolve complaints directly with the cable television provider prior to contacting the city. You can reach the cable television providers at:

  • Cox: 623-594-1000
  • CenturyLink: 602-266-1700

If you have questions, comments or complaints about the providers, please complete and submit this online form. It is pre-addressed to arrive at the correct destination when you click on the Submit button below.
